Home Care Package Program

ACI Home Care are specialists in providing Home Care Package (HCP) services. We partner with approved home care providers to deliver coordinated packages of care and services that help people with more complex care needs to remain living independently at home.

There are four levels of Home Care Packages to help meet different care needs. During your aged care assessment, your assessor will discuss your needs and find out which package level is best to meet them.

  • Level 1 - basic care needs

  • Level 2 - low care needs

  • Level 3 - intermediate care needs

  • Level 4 - high care needs

Are you eligible for subsided home care?

To be able to receive subsidised home care services you must first register with My Aged Care and have an aged care assessment. My Aged Care is the starting point to apply for all Australian Government-funded aged care services.

You can register yourself, or you can ask a family member, friend, doctor or hospital to refer you. If you still need assistance, you can talk with one of our advisors who can assist you with the process.

Private clients

If you are not eligible for subsidised support, you can still access ACI Home Care as a private client for a special rate. Please contact us to for more information.

NDIS

The NDIS supports people who have a permanent and significant disability. Some NDIS participants decide to continue their NDIS plans into their later years.

If you are an older NDIS participant, our sister company Sydney Care Partners is a registered NDIS provider and can assist you.
